how do i join the plan?

Our dental plan is delivered at three levels as detailed opposite. Your dentist will advise you which level of care is best suited to your needs. Then all you have to do is complete a Direct Debit mandate and registration form. In addition to your first monthly payment, a one-off registration fee of £10 will be collected with your first direct debit payment. If you choose to end your plan you can do simply by giving us one months notice.
